Retrograde Ejaculation. The backward flow of semen into the bladder instead of down the
penile urethra generally results from a failure of the bladder neck to approproiately close during
ejaculation. This condition can have several causes, including idiopathic, diabetic related,
postsurgical (RPLND or V-Y plasty of bladder neck). Neurogenic and idopathic forms can often
(30%) be treated with alpha-sympathomimetic agents: Imipramine 25-50 mg b.i.d, Nasal-D b.i.d.,
Ornade b.i.d., or Sudafed plus (60mg) q.i.d. If this fails, sperm harvesting techniques can often be
used with intrauterine insemination to achieve a pregnancy.
