If your cat has been to the vet within one year most often you can request the medication to treat the worms over the counter- without an exam. You want something like Droncit to treat tapeworms or Strongid-T for the roundworms.
Now the eye thing you will most likely have to be seen for... Here in California an exam at my clinic is $42. Then you have whatever diagnostics they do to tell what is causing it and then probably meds to put in the eye and if it's due to an upper respitory infection (quite common) maybe some antibiotics. At my clinic you may be paying ~$100.
