do you really need a doctor to sign for you? can't you just get ssdi? their dr has to check you out anyways. So just get the paper work and have their dr look at you we have ddd which is a disability adviocate basically and once you get in there it is pretty easy to get ssdi as long as you have work credits.
I would get in touch with the social services http://www.ssa.gov/disability/ well i am sure you have or just get a new dr or something because what you CAN collect on ssdi is usually worth the pain in the ass that it is to collect it
