If the SO is not someone you know well, like what I'm getting in the other cases, I don't think you have any duty to say anything.  But when it's someone you love and who trusts you, I don't see it as a choice.  I think it's just the right thing to do.  I'm sure it's incredibly hard, but you need to help out a family member.  She may be angry and take it out on you, but then again, she might be grateful in the long run.  Since it's about a family member, you won't run into TMax's second issue.
