Not sure if this will help your fear, but because of my older brother and sister, I have a huge fear of snakes, bugs and spiders. I also suffer from a medium to sever case of claustrophobia. Unfortunately with  my former occupation, I was required to go in attics and crawl spaces where there was a big lack of space and them bugs and crap. What I did to overcome these when working was force myself into a meditative state that I learned from getting my ink to shut out the pain (Which I use daily to counter the pain from my injuries so I can live a somewhat normal life), but of course it only lasted until I got out from the crawl spaces and bugs.. Then... I would have to decompress and get rid of the shivers.  Don't know if you would be able to do that when in your situation, but that's what helps me.  Are you able to go out on boats at all? If not, this might work in that situation.
