Is the clientele in the restaurant mainly locals or tourists?

Tipping wasn't a big thing in England, people would leave 8-10% on average in decent places. However, in Italy it's such a foreign concept to people. I was in a really nice restaurant the other night, in a very affluent neighbourhood and I saw someone leave 2euros. They're bill must have easily exceed 200euros.

How does it generally work there?

you generally double the tax and round up in the US, or do 10%. I was in london over the summer and its common to not tip, most places add it to your bill for you if its expected.
