Ahhh good old Josh. The portrait in question as well as many others that feature this doll that dont make any fucking sense are taken from a series called "Understanding Joshua". Joshua is meant to be a physical embodiment of male insecurities. He's ugly, got no dick, big feet, awkward body, going bald. There are others in the series where you have people interacting with just chunks of his flesh.

For future reference if you see a photo and you wonder what the fuck is going on, odds are it's one of Charlie's works.
