You probably haven't heard of it (or him) because it didn't get a wide release, and it did not sell well at all. (He partially blames it on the fact that his publisher decided to go ahead and do a hardcover release, despite the book being aimed at a crowd that does not generally have the money or the desire to buy hardcover.)
At this point, odds are you can only buy it directly from him (his site), or possibly used on Amazon.
The opening paragraph:
The day I turned sixteen years old I had no idea that in four months nearly everyone I cared about would be dead. Unburdened by this foreknowledge, it was with a free and unclouded spirit that I went down to the DMV and failed my driving test.
He's also written some lovely interactive fiction, and some shorter pieces, all of which can be found on the above-linked website.
