Name some movies that you find really unsettling not because they are especially gory or violent, or because of the horrible things that happen to the characters, but rather because the elements of the film contrast in a really dissonant, upsetting way.

Four I can think of, in order:

1. Meet the Feebles
2. The Garbage Pail Kids
3. Radio Flyer
4. The Devil's Advocate

When I was a kid, the way the lighter sequences in E.T. contrasted with E.T.'s suddenly dropping dead in a medical tent gave me a similar feeling.

Yup that's Audition, famed for swtiching genres from arthouse to gore-fest halfway through, undercutting all expectations. One of the few films I actually couldn't sit through.

Keeping to Jeff_Fries's characteristics, I'd mention most of David Lynch's work. Mulholland Drive, Lost Highway and even Rabbits have these unsettling music and oniric scenes that give you the creeps.
On the oriental side, I'd mention Darkwater (Honogurai no Mizu no Naka kara). Nothing really happens -till it happens- but the humidity is certainly disturbing.

Yeah, the humidity.

Henry: portrait of a serial killer. I'm pretty much de-censortised to most graphic film's but this was too much. I couldn't hack it shocked

i just rented garbage pail kids for the first time. my concern is not the kids gross out features, but the fact that the half assed puppetteering left them bobbing and dancing with vacant unfocused eyes. jokes about gross little kids playing doctor are a little unsettling.

im going to do a half vote for gremlins. what is it, a horror movie or a cutesy movie? while its never truly unsettling, it does lull you for a while as to whether these things are funny or dangerous. as a kid it disturbed me, particularly the futterman's plow sequence.

resevoir dogs probably belongs on the list... that ear scene sounds like the definition of what youre talking about.

refresh my memory, whats wrong with radio flyer? the realistic portrayal of the abusive dad?

There's nothing realistic about it. It's a failed attempt to make a Spielbergesque story about two kids escaping the physical abuse of their stepfather.

Either one of the kids flies away on his homemade airplane, or he died in the attempt and his brother has grown up deluding himself into believing that he's still flying and traveling the world, sending him postcards from the places he visits. What the hell are they trying to say?

Irreversible doesn't quite fit your criteria, but the opening scene of the film, as well as the rape scene give me the heeby jeebies. Very uncomfortable.

Adaptation. I can't put my finger on it- maybe pity for Charlie, or perhaps it's the super creep factor of Chris Cooper's character, but that one makes me feel awkward and depressed every time.
