Ohio fucked with my license all the time. First they put on a restriction that I didn't have and it took some time trying to correct the fact that I DID NOT have a medical restriction. Then they LOST my license and made ME pay for a replacement.
To get my license in MI I had to have 3 forms of ID plus a bill at my address. Note. Social Security cards don't count as ID, but previous car registrations do and school transcripts do. 
I also remember hearing tales of horror from the former SG Red Violet where her PA license expired before she went to get her OH one so they made her do a learners permit. She had to have licensed adult in the car with her at all times while she drove...at the age of 25!
Ohio sucks.
