If that's true, then wouldn't harping on those topics simply exacerbate the problem? I mean, I'm not saying she'll ever see or hear his comments, but in general, I think it's the kind of comments that unattractive people receive, rather than simply being unattractive, that have the potential to give them a bad attitude.

Not really, as in reality, people are not likely to just say to some random stranger "Goddamn, you're fat!", it's just not the done thing. I'd say 99% of their attitude is created themselves, based on their own assumptions of people's opinions towards them, and that just creates an ongoing cycle: They treat people like shit, to make themselves feel better, and because they're being nasty to someone, well, that person serving them is not gonna be nice back, but the person thinks that the other is judging them by their size or looks, when in reality, they're being judged by their attitude and behaviour.
As someone who spent the last 13 years (until I spoke to Debra Jean) hating the sight of myself, I understand that mindset rather well.
