Duchovny was in more than one episode. In fact, he was the narrator for the series, so technically he was in every episode, though he never got it on except for in the initial movie.

Depends on which softcore films you are talking about. The ones that fill up air time on Cinemax and Showtime now are mostly dual productions (shot at multiple angles, as someone mentioned above) which they may shoot a few extra scenes of dialogue for to compensate for the cut out hardcore footage.
If you are talking about the stuff you might find in this guy or this guy, or this guy's filmography, well, being a friend of someone involved is a pretty big help. There are lots of producers who churn out softcore flicks by paying maybe 5 actors/actresses/models their fee and filling in the other parts, cast and crew, with buddies they have made in and out of the business. Most of the guys are washouts who can't even get on a soap opera set as an extra and are happy to take any work they can get for nothing, or they are someone's buddy.

Damn, I haven't had Cinemax in a good long while, but when did that shit start happening? I can't imagine much worse softcore than edited hardcore.
